IMPORTANT INFORMATION FOR STUDENTS REGARDING VACCINATIONS AND HEALTH EXAMS

1. All students in grades 7th - 12th will need to receive the Tdap (Whooping Cough) shot in order to be able to enroll at MCHCS for the 2012-2013 School Year. This is a state law that was effective beginning July 1, 2011. Proof of the vaccination must be brought to the school prior to enrollment. (This only applies to new 7th graders and any 8th-12th graders that have not already brought in proof of having this vaccination.)

2. Incoming Kindergarten students must have a physical exam done and be up to date on all of their vaccinations.  A copy of this information and as well as a birth certificate is required at the time of enrollment.
